Brasil Sorridente: National Oral Health Policy

Problem
Oral health care in the SUS. The guidelines of the National Oral Health Policy were published by the Ministry of Health in 2004; in 2023, Law No. 14,572, which according to Senator Jussara Lima arose from a bill by Senator Humberto Costa, instituted the policy in the SUS and included oral health in the SUS's field of action in Law 8,080/1990.
Who it serves
According to the Ministry of Health, one should go to the nearest UBS (basic health unit) with an identification document, the SUS card, and proof of residence.
How it works
Dental Specialty Centers and Regional Dental Prosthesis Laboratories accredited by the Ministry of Health based on a proposal from the municipal manager approved by the Bipartite Intermanagerial Commission (Ordinance GM/MS No. 1,570/2004). According to the Ministry, access begins with the Oral Health teams of the Basic Health Units, which refer patients to the CEO when specialized care is needed. According to Senator Jussara Lima, the dental surgeon had already been included in the Saúde da Família teams in 2000.

Health

Relation to the Lula governments

Reshaped under Lula · First term (2003 to 2006) · since 2004

The 'Guidelines of the National Oral Health Policy' were published by the Ministry of Health (Health Care Secretariat, Primary Care Department, National Oral Health Coordination) in 2004. The presentation is dated January 2004 and signed by Humberto Costa, Minister of State for Health, with Jorge Solla, Secretary of Health Care, Afra Suassuna, Director of the Primary Care Department, and Gilberto Alfredo Pucca Jr., National Oral Health Coordinator.[1]

Ordinance GM/MS 1,570/2004 defined the CEOs as establishments that provide, at a minimum, oral diagnosis with an emphasis on oral cancer, specialized periodontics, minor oral surgery, endodontics, and care for people with special needs; and the LRPDs as services for complete and partial removable dental prostheses.[2]

Reshaped under Lula · Third term (since 2023) · since May 2023

Law No. 14,572 of May 8, 2023, signed into law by Luiz Inácio Lula da Silva, instituted the National Oral Health Policy in the SUS and amended Law 8,080/1990 to include oral health in the SUS's field of action.[3]

Origin and precursors

Origin milestone: founding legal act, First term (2003 to 2006).

Precursors

  • Inclusion of dentists in Family Health (Ordinance 1,444/2000), according to a parliamentary speech earlier technical basis According to Senator Jussara Lima, who chaired the 69th Meeting of the Senate Social Affairs Committee, at a hearing on November 27, 2025, the dental surgeon was included in the Saúde da Família teams in 2000, by Ordinance GM/MS No. 1,444 of December 28, 2000, which establishes a financial incentive for reorganizing oral health care in the municipalities; and Ordinance GM/MS No. 267 of March 6, 2001 approved rules and guidelines for including oral health in the PSF.[4]
  • Reinforcement of the oral health teams of the Family Health Strategy earlier technical basis In the same remarks, the senator states that the 2004 guidelines of the new National Oral Health Policy included reinforcing the inclusion of oral health teams in the family health strategy and the creation of the Dental Specialty Centers. The MS guidelines document (2004) contains the item 'Incorporate Saúde da Família as an important strategy in the reorganization of primary care' and records that the Saúde da Família strategy was created in 1994.[5]
  • Ordinance 74/2004 earlier legal basis Ordinance GM/MS 1,570/2004 mentions, in its recitals, Ordinance No. 74/GM of January 20, 2004, which indicates the need for support from a network of specialized dental services.[6]

Precursor investigation: inconclusive (October 13, 2026).

Scope: Norms and statements of reasons at the Planalto, publications of the ministries and oversight bodies, searches recorded in the dossier (research of October 13, 2026). Specialized care (CEO and prosthesis laboratories) is new; in primary care there is continuity. The 2000 antecedent comes from a parliamentary speech, not from the ordinance (404 at the BVS). Act that creates the name "Brasil Sorridente" not located.

Responsibilities

  • Municipalities, bipartite commissions, and Ministry of Health municipal implementation

Under Ordinance GM/MS 1,570/2004, the municipal manager submits the CEO or LRPD proposal to the state's Bipartite Intermanagerial Commission (CIB); the CIB informs the Primary Care Department of the MS of the approved units, and the Department reviews the request; it is up to the Minister of Health to approve and formalize the accreditation of the CEOs, limited by the MS's financial availability.[7]

  • Public units and universities (CEO providers) partnership

Under Ordinance GM/MS 1,570/2004, only health units with a public legal nature and universities of any legal nature may be accredited as CEOs; for LRPDs there is no restriction on legal nature.[8]

  • National Congress (Law 14,572/2023) legislative approval

According to Senator Jussara Lima, Law No. 14,572/2023, which instituted the National Oral Health Policy in the SUS, arose from a bill by Senator Humberto Costa.[9]

How it works

According to the Ministry of Health, access to oral health actions in the SUS begins with the Oral Health teams of the Basic Health Units and the Mobile Dental Units; when specialized care is needed, the team refers the user to the Dental Specialty Centers (CEO); the Regional Dental Prosthesis Laboratories (LRPD) carry out the laboratory stage of making the prostheses.[11]

The Brasil Sorridente frequently asked questions page (updated August 25, 2025) reports monthly operating funds of R$23,100.00 for Type I CEOs, R$30,800.00 for Type II CEOs, and R$53,900.00 for Type III CEOs, transferred fund to fund.[12]

Who it serves

To access the program, the Ministry of Health advises bringing an identification document, the SUS card, and proof of residence and going to the nearest UBS (basic health unit) to schedule an appointment with the oral health team.[13]

Budget and spending

Figures are nominal unless stated otherwise. Announced, authorized, committed, liquidated and paid amounts are different things and appear with the type indicated. In SIOP lines, "paid" means paid within the same fiscal year (arrears settled later are not counted), and one budget action can fund more than one program: the text says when the action is only part of a program.

In the SIOP, nominal values: in 2006, action 6838 (Oral Health Care) had an authorized appropriation (law plus credits) of R$109,175,749, a commitment of R$84,746,488, a liquidation of R$84,746,488, and a payment in the fiscal year of R$49,537,261; in 2010, action 8730 (Primary Oral Health Care) had an authorized appropriation (law plus credits) of R$56,142,062, a commitment of R$38,882,108, a liquidation of R$38,882,108, and a payment in the fiscal year of R$2,244,270; and in 2010, action 8934 (Specialized Oral Health Care) had an authorized appropriation (law plus credits) of R$89,296,102, a commitment of R$85,513,827, a liquidation of R$85,513,827, and a payment in the fiscal year of R$75,739,810.[14]

Deliverables

Announcement, contracting, completion and operation are different phases.

According to the Ministry of Health's frequently asked questions, at the end of 2012 there were 22,139 oral health teams operating in 4,907 municipalities, and between 2004 and 2012 943 CEOs were implemented in 771 municipalities.[15]

In the Ministry of Health's open data (Oral Health Care Network dataset), the number of Dental Specialty Centers implemented and funded in Brazil (column vl_indicador_calculado_br) was 1,087 in December 2016, 1,158 in December 2022, 1,186 in December 2025, and 1,243 in June 2026.[16]

In the Ministry of Health's open data (Oral Health Care Network dataset), the number of Oral Health teams (40 hours plus differentiated workload) in Brazil (column vl_indicador_calculado_br) was 24,384 in December 2016, 28,253 in December 2022, 34,936 in December 2025, and 35,613 in September 2026.[17]

Audits

In Evaluation Report No. 1558750, the CGU recorded that the minimum targets currently set by the Ministry of Health for the different types of CEO are not being met by 96.6% of the sample of municipalities and the Federal District (28 of 29 federative entities inspected), although all of these units continue to receive the federal public funding for operating costs every month; in the CGU's view, the minimum production rules set by the Ministry may be excessive and need to be changed.[18]

Limitations

According to CGU Evaluation Report No. 1558750, the PNSB is operationalized through a network that brings together more than 40 thousand eSB (oral health teams), in addition to 1.2 thousand CEOs, with federal investment of approximately R$4.3 billion in 2024; the current coverage of about 45% corresponds to approximately 91 million people, against the government target of reaching at least 70%.[19]

According to the article by Pucca Jr. et al. (Journal of Dental Research, 2015), a descriptive assessment of the policy's first 10 years, the most significant results appear in the epidemiological indicators, in investment, and in the building of the care network, but the oral health network still needed to be expanded, improved, and consolidated so that the policy would endure and not be the action of a single government.[20]

Context

According to the Ministry of Health, Brasil Sorridente is a dental care program created in 2004, integrated into the National Oral Health Policy.[21]
